The goal of this course is to equip participants with necessary information about the role of patient care management to reduce hospital readmissions specifically among patients in caregiver settings. This course examine and review some of the strategies to reduce the likelihood of unplanned readmission and adverse events after the patient is discharged from the hospital to a home health care or assisted living setting. This course is for everyone who is involved in any stage of patient care; hospital admission process, discharge process, transitional care planning, and patient’s recovery care post discharge.
